If you are a California citizen whose Private Information was disclosed by Siskiyou Hospital, Inc. d/b/a Fairchild Medical Center (“Fairchild Medical Center”) to third parties through Meta Pixel and related technologies without authorization, which totals approximately 1,000 individuals, then you may be a Class Member. Excluded from the Settlement Class are: (1) the Judge in this case, and the Judge’s family and staff; (2) Fairchild Medical Center and its officers, directors, and related companies; (3) anyone who validly excludes themselves from the Settlement; (4) the legal representatives, successors or assigns of any such excluded persons; and (5) attorneys for the parties.
If you are a Settlement Class Member and wish to receive Settlement Benefits then you must submit a Claim Form by November 2, 2026.
All Settlement Class Members may enroll in a year of CyEx Privacy Shield Pro and/or claim the cash payment option. The benefits are explained in more detail below.
CyEx Privacy Shield Pro. All Class Members are eligible to enroll in one (1) year of CyEx Privacy Shield Pro. Enrollment codes have been sent to all Class Members by postcard. Cash Payment. Class Members may also claim a one-time cash payment. This payment is expected to be $25.00.
To start your online claim form, you must login with your LoginID and PIN. You can find your LoginID and PIN on the Notice that was sent to you. If you are unable to locate your ID and PIN, please contact us by emailing info@FairchildDataSettlement.com. Please request your LoginID and PIN by providing your full name and mailing address.

The deadline to submit a Claim Form online is November 2, 2026. If you are mailing your Claim Form, it must be mailed with a postmark date no later than November 2, 2026.
